The transition from an acute care hospital to home can take time. Southampton Memorial Hospital’s Skilled Nursing Care Facility provides continued care for people going through this change. Here, our Skilled Care team provides the additional care these patients need to get to a place where they are ready and prepared for a successful return home.
We offer the following services (and more) for those needing additional care when recovering from medical procedures and surgeries:
While in Skilled Care, patients are encouraged to dress in their own street clothes. Meals are served in the patient’s room, and family members may purchase a guest tray to be delivered so that they may eat with their loved one.
Our Activity Coordinator offers patients various opportunities for staying active, including reading newspapers, solving puzzles, making crafts, playing games and other activities. We encourage families to take their loved one outside if the weather is favorable. Families may also take patients, as health conditions permit, to visit the East Pavilion, our long-term care facility. Passes to go off the campus are permitted with a physician order.
