Home Health Home health care is not an option for everyone. Generally, the person receiving care or treatment at home must be well enough to be cared for in the home setting. |
Southampton Memorial Home Health Services is a comprehensive accredited home health program. As a hospital-based program, we offer services by a professional staff that helps smooth the transition from the hospital to the home. Our program is accredited and Medicare/Medicaid certified.
To receive our home health care services, a referral must be made by your doctor who develops and approves your individualized case management plan. We take referrals from all sources, even family members.

Hospice Services
Hospice is a special way of caring for people with a terminal illness and their families. Receiving a diagnosis of a serious, possibly terminal illness, may result in different feelings in the patient and the family members. Each may have his or her own way of coping during this difficult time.
Southampton Memorial Hospice offers physical, emotional, and spiritual help to the patient and the family so that the patient may remain at home. Our goal is to be a resource of information and support for both the patient and the family so that each day can be lived with dignity and comfort.
